Background: In Switzerland, non-medical right-to-die organisations such as Exit Deutsche Schweiz and Dignitas offer suicide assistance to members suffering from incurable diseases. Objectives: First, to determine whether differences exist between the members who received assistance in suicide from Exit Deutsche Schweiz and Dignitas. Second, to investigate whether the practices of Exit Deutsche Schweiz have changed since the 1990s. Methods: This study analysed all cases of assisted suicide facilitated by Exit Deutsche Schweiz (E) and Dignitas (D) between 2001 and 2004 and (...) investigated by the University of Zurich’s Institute of Legal Medicine (E: n = 147; D: n = 274, total: 421). Furthermore, data from the Exit Deutsche Schweiz study which investigated all cases of assisted suicide during the period 1990–2000 (n = 149) were compared with the data of the present study. Results: More women than men were assisted in both organisations (D: 64%; E: 65%). Dignitas provided more assistance to non-residents (D: 91%; E: 3%; p = 0.000), younger persons (mean age in years (SD): D: 64.5 (14.1); E: 76.6 (13.3); p = 0.001), and people suffering from fatal diseases such as multiple sclerosis and amyotrophic lateral sclerosis (D: 79%; E: 67%; p = 0.013). Lethal medications were more often taken orally in cases assisted by Dignitas (D: 91%; E: 76%; p = 0.000). The number of women and the proportion of older people suffering from non-fatal diseases among suicides assisted by Exit Deutsche Schweiz has increased since the 1990s (women: 52% to 65%, p = 0.031; mean age in years (SD): 69.3 (17.0) to 76.9 (13.3), p = 0.000), non-fatal diseases: 22% to 34%, p = 0.026). Conclusions: Weariness of life rather than a fatal or hopeless medical condition may be a more common reason for older members of Exit Deutsche Schweiz to commit suicide. The strong over-representation of women in both Exit Deutsche Schweiz and Dignitas suicides is an important phenomenon so far largely overlooked and in need of further study. (shrink)

This chapter explores the concept of time itself, as physicists understand it. Einstein's special theory of relativity requires worldlines of physical objects to be timelike; the field equations of his general theory of relativity predict that massive bodies such as stars and black holes distort space‐time and bend worldlines. Suppose space‐time becomes so distorted that some worldlines form closed loops. If one tried to follow such a closed timelike curve (or CTC) exactly, all the way around, one would bump into (...) ones former selves and get pushed aside. But by following part of a CTC, we could return to the past and participate in events there. According to a recent calculation by J. Richard Gott of Princeton, a cosmic string passing rapidly by another would generate CTCs. The chapter concludes that if time travel is impossible, then the reason has yet to be discovered. (shrink)

A major problem facing no-collapse interpretations of quantum mechanics in the tradition of Everett is how to understand the probabilistic axiom of quantum mechanics (the Born rule) in the context of a deterministic theory in which every outcome of a measurement occurs. Deutsch claims to derive a decision-theoretic analogue of the Born rule from the non-probabilistic part of quantum mechanics and some non-probabilistic axioms of classical decision theory, and hence concludes that no probabilistic axiom is needed. This book is a reprint of one of the pioneer works on German mysticism of the nineteenth century. It is a comprehensive account of the most fertile hundred years of German spiritual and mystical history in the Middle Ages. In contrast to Bach's and Lasson's books on Eckhart written in the same decade, Greith's viewpoint is one of narrow scholastic orthodoxy. However, the wealth of detail and the pleasant simplicity of style compensate for those rather irritating lamentations about the "errors" (...) of Meister Eckhart. Although this book was written before Denifle's discovery of Eckhart's Latin works, it remains of value today due to its systematic study of the doctrinal positions of the major mystical writers of the Rhineland. Included as well is an interesting analysis of mystical poetry centered around the works of the famous Sister Mechtild. The book ends with a lengthy and detailed description of the spirituality and spiritual theory developed in the scattered nunneries of the preaching orders.—M. J. V. (shrink)

Neither the speculative depth of Meister Eckhart nor the willpower and inner drive of Tauler are to be found in the writings of the third great mystic of fourteenth-century Rhineland, Henry Seuse. But we might well be compensated by authenticity of the description of the spiritual experience. This is not an edition for scholars and does not even try to resurrect the so savagely fought for issue on the authorship of Seuse's autobiography. We have here a major document of spiritual (...) life, with a great literary work making up about one-half of it. The other half contains the Booklet of the Eternal Wisdom, the Booklet of Truth, the Booklet of Letters, and two Sermons. Perhaps a little more critical apparatus and a little more comment on the themes of the texts would have improved the book, but instead of giving this, the translator simply refers to a book on Henry Seuse published in the same year, 1966. In this paper, we develop the notion of a natural convention, and illustrate its usefulness in a detailed examination of indirect requests in English. Our treatment of convention is grounded in Lewis’s seminal account; we do not here redefine convention, but rather explore the space of possibilities within Lewis’s definition, highlighting certain types of variation that Lewis de-emphasized. Applied to the case of indirect requests, which we view through a Searlean lens, the notion of natural convention allows us to give (...) a nuanced answer to the question: Are indirect requests conventional? In conclusion, we reflect on the consequences of our view for the understanding of the semantics/pragmatics divide. (shrink)

Comparative or cross-cultural philosophy can be seen as a relative newcomer to the field of philosophy. It has its antecedents in the emergence of comparative studies in nineteenth-century European intellectual history, as well as in the sequence of East-West Philosophers’ Conferences at the University of Hawaii, which began in 1939. This book will prove to be of great significance in helping to define a field that is only now becoming fully self-conscious, methodologically and substantively, about its role and function in the larger enterprises of philosophy and comparative studies. Recent research into the evolution of higher cognition has piqued an interest in the effect of natural selection on the ability of creatures to respond to their environment (behavioral plasticity). It is believed that environmental variation is required for plasticity to evolve in cases where the ability to be plastic is costly. We investigate one form of environmental variation: frequency dependent selection. Using tools in game theory, we investigate a few models of plasticity and outline the cases where selection would (...) be expected to maintain it. Ultimately we conclude that frequency dependent selection is likely insufficient to maintain plasticity given reasonable assumptions about its costs. This result is very similar to one aspect of the well-discussed Baldwin effect, where plasticity is first selected for and then later selected against. We show how in these models one would expect plasticity to grow in the population and then be later reduced. Ultimately we conclude that if one is to account for the evolution of behavioral plasticity in this way, one must appeal to a very particular sort of external environmental variation. (shrink)

It is generally agreed that the current methods of providing health care in the West need to be reformed. Such reforms must operate within the practical limitations to which any future system of health care will be subject. These limitations include an increase in the demand for costly end-of-life health care coupled with a reduction in the proportion of the population who are working taxpayers (and hence a reduction in the proportionate amount of health care funding that can be secured (...) through taxation) and the fact that the imposition of bureaucratic regulations on health care systems is costly. Recognizing these limitations should naturally lead one to consider market-based reforms. Yet despite the practical impetus for such reforms, there is still widespread concern that market-based health care is unethical. The purpose of this paper is to address this concern and, in so doing, to pave the way for the market-based reform of health care to proceed. (shrink)

One of the most common arguments against legalising markets in human kidneys is that this would result in the widespread misuse that is present in the black market becoming more prevalent. In particular, it is argued that if such markets were to be legalised, this would lead to an increase in the number of people being coerced into selling their kidneys. Moreover, such coercion would occur even if markets in kidneys were regulated, for those subject to such coercion would not (...) be able to avail themselves of the legal protections that regulation would afford them. Despite the initial plausibility of this argument, there are three reasons to reject it. Firstly, the advantages of legalising markets in human kidneys would probably outweigh its possible disadvantages. Secondly, if it is believed that no such coercion can ever be tolerated, markets in only those human kidneys that fail to do away with coercion should be condemned. Finally, if coercion is genuinely opposed, then legalising kidney markets should be supported rather than opposed, for more people would be coerced were such markets to be prohibited. (shrink)

The introduction of the Diagnostic and Statistical Manual of Mental Disorders in May 2013 is being hailed as the biggest event in psychiatry in the last 10 years. In this paper I examine three important issues that arise from the new manual: Expanding nosology: Psychiatry has again broadened its nosology to include human experiences not previously under its purview. Consequence-based ethical concerns about this expansion are addressed, along with conceptual concerns about a confusion of "construct validity" and "conceptual validity" and (...) a failure to distinguish between "disorder" and "nondisordered conditions for which we help people." The role of claims about societal impact in changes in nosology: Several changes in the DSM-5 involved claims about societal impact in their rationales. This is due in part to a new online open comment period during DSM development. Examples include advancement of science, greater access to treatment, greater public awareness of condition, loss of identify or harm to those with removed disorders, stigmatization, offensiveness, etc. I identify and evaluate four importantly distinct ways in which claims about societal impact might operate in DSM development. Categorisation nosology to spectrum nosology: The move to "degrees of severity" of mental disorders, a major change for DSM-5, raises concerns about conceptual clarity and uniformity concerning what it means to have a severe form of a disorder, and ethical concerns about communication. The dominance in normal awareness of visual percepts, which are linked to space, obscures the fact that most thoughts are non-spatial. It is argued that the mind is intrinsically non-spatial, though in perception can become compresent with spatial things derived from outside the mind. The assumption that the brain is entirely spatial is also challenged, on the grounds that there is a perfectly good place for the non-spatial in physics. A quantum logic approach to physics, which takes non-locality as its (...) starting point, offers a non-reductive way of reconciling the experience of mind with the world description of physics. For further progress it is necessary to place mind first as the key aspect of the universe. (shrink)

As part of a larger research study, we present model language for disclosing financial interests in clinical research to potential research participants, and we describe the empirical basis and theoretical assumptions used in developing the language. The empirical process for creating appropriate disclosure language resulted in a generic disclosure statement for cases in which no risk to participants’ welfare or the scientific integrity of the research is expected, and nine more specific disclosure statements for cases in which some risk is (...) expected. The disclosure statements are not meant to be canonical, but were instead designed to reflect the typical situations in which disclosure of financial interest might be considered by an institutional review board or conflict of interest committee. Individual institutions could modify key phrases to suit their purposes, and others could use the language in future empirical work on informed consent to better refine the options for disclosing financial interests in clinical research.
